PER MONTH
One annual preventive exam plus up to four additional in-office primary care visits.
Additional in-office primary care visits during the same membership year are $50 per visit.

YOUR MEMBERSHIP INCLUDES
Primary care coverage for routine needs.
Services are limited to the primary care items described in the membership agreement and are provided during each 12-month membership period.

Additional medically indicated laboratory testing may be available at negotiated discounted self-pay laboratory rates. Pricing depends on the specific tests ordered.
IMPORTANT
This membership is not health insurance.
It is not a substitute for health insurance. It covers only the primary care services specifically described in the membership agreement. Patients should consider maintaining appropriate health coverage for services and expenses outside the membership.

Who is this program for?
Eligibility is based on self-pay status at this Practice. Uninsured patients may enroll. Patients with commercial insurance that EFMD does not accept may be eligible after verification.
Can I have insurance and still participate?
Possibly. The program is based on self-pay eligibility at EFMD, not simply on whether you have an insurance card. Coverage with special billing requirements requires administrative review.
Are telehealth visits included?
No. The membership is designed around in-office primary care.
Do unused visits carry over?
No. They do not accumulate, transfer, or carry forward and have no cash value.
What happens if a monthly payment fails?
You have a 10-calendar-day grace period to correct the failed payment. The grace period does not change your original membership or renewal dates.
Can I cancel?
Yes. Either party may terminate under the written notice provisions of the Agreement, generally with at least 30 days’ advance written notice.
Appointments
Membership does not guarantee immediate appointments. Scheduling remains subject to clinical need and Practice availability.
Emergencies
Call 911 or go to an appropriate emergency facility. Healow is not for emergencies.
